Participating in High School Athletics
All students are required to complete medical forms (see form explanation letter) which must be on file in the school’s Athletic Department prior to the first official day of their sport practice.

Download Medical Forms or pick them up at the CFC office. Please return all required forms (together) to the CFC/Athletic Department when completed.

Eligibility Requirements
Perry’s eligibility requirement is a minimum 2.0 GPA each six weeks in order to remain eligible for all extracurricular participation. In addition, the “no pass, no play rule” continues to be in effect. Any failing grade will result in ineligibility for the next six weeks. (See Student-Athlete Handbook.) Parents are encouraged to monitor interim reports carefully!
